Once you have applied to Surry Community College, follow these four Health Sciences specific admissions steps.
A student’s admission to the Health Science programs is based upon a comparison of grades and entrance test ranking with other applications. Admission into the Physical Therapist Assistant (PTA) program will also be based on a personal interview, essay, and completion of clinical observation hours.

Fall ADN students, who take the admission test in February, must turn in all application materials by February 15. Students who take the admissions test in May must turn in all materials by May 15.
Spring PN and LPN-ADN students, who take the admission test in June, must turn in all application materials by June 15. Students who take the admissions test in September must turn in all materials by September 15.
Summer LPN-ADN students, who take the admission test in December, must turn in all application materials by November 15. Students who take the admissions test in March must turn in all materials by February 15.
Fall PTA students must turn in all materials by May 31.
Summer LPN-BSN students must turn in all materials by November 15 for early admission, and by February 15 for final admission.
Fall RIBN students must turn in all materials by February 15 for early admission, and by May 15 for final admission.
Reading, English, Science and Math.
The ATI TEAS Test is the admissions test used for admission to the Physical Therapy Assistant program and the Nursing programs at Surry. The TEAS measures comprehension and skills in Math, Reading, Science and English. Students are only eligible to take the TEAS test once every 3 months. Taking earlier versions of the test currently used at Surry Community College, or at different locations will be counted in the frequency of testing, but no other versions or scores will be accepted. TEAS scores will remain valid for one year.
Surry requires students to take a different version of the ATI TEAS test every 3 months; therefore, students are encouraged that all testing be completed at Surry. Students can take the test at any location and have the official transcripts from Assessment Technologies Inc. (ATI) sent to Surry, Attn: Associate Dean of Health Sciences.
These tests are offered by the college in February and May for fall ADN, RIBN, and LPN-BSN candidates, February and May for fall PTA candidates, June and September for spring PN and LPN-ADN spring-start candidates, and December and March for LPN-ADN summer-start candidates.
Students who wish to take the test should contact the Health Sciences Instructional Assistant in H-314. Students must pre-register for the TEAS by contacting Carol Roberts, Instructional Assistant for Health Sciences, at (336) 386-3275 or robertscd@surry.edu Cost of the PSB is approximately $85.00, and cost for the ATI TEAS Test is approximately $65.00. Fee for the admissions tests are due at the time of registration. Study guides and online practice tests may be purchased at John A. Logan College Testing Services for the PSB. Students taking the TEAS can purchase a study guide from ATI Testing.
Students taking the test on the computer will receive their score at the conclusion of the test and will know if they met the benchmark score required for qualification as a possible candidate for admission. The numeric score benchmark for the TEAS for Nursing is 60% overall with no subtest measuring less than 25%. The numeric score benchmark for the TEAS for PTA is 59.4%. Once the admissions tests have been completed, the Associate Dean of Health Sciences will provide Student Services with a list of students who have met the benchmark score on the TEAS. An audit of each student’s file will be conducted to ensure all criteria for admission have been met.
Admission into the Health Science programs is contingent upon completion of the following items by the dates requested in the Nursing or PTA Applicant Information Session.

There are two opportunities to be considered for program admission each fall into the ADN program. For ADN students, an early admission process for students who meet all criteria and take the TEAS test by the February cutoff date, and a final admission process for students who meet all criteria and take the TEAS test by the May cutoff date. There is one opportunity to be considered for program admission each fall into the PTA program for students who meet all criteria and take the TEAS by the May cutoff date.
Each fall those student applicants who have the highest rank on their grades earned in BIO 168, BIO 169, ENG 111, ENG 112 or ENG 114 and MAT 152 (current High School students in top 15% of class receive ten points) and the PSB will be admitted into the ADN program.
For LPN and spring LPN-to-ADN, an early admission process for students who meet all criteria and take the TEAS by the June cutoff date, and a final admission process for students who meet all criteria and take the TEAS by the September cutoff date. For summer LPN-to-ADN, an early admission process for students who meet all criteria and take the TEAS by the November cutoff date, and a final admission process for students who meet all criteria and take the TEAS by the February cutoff date. Student applicants who have the highest rank in BIO 163, ENG 111, PSY 150 (high school students in the top 15% of class) will be admitted into the PN program. Student applicants who have the highest rank in BIO 168, BIO 169, ENG 111, ENG 112 or ENG 114, and MAT 152 will be admitted into the LPN-ADN program.
In Phase I of the admission process students will be ranked based on their GPA, grades earned in BIO 168, BIO 169, ENG 111, PSY 150, PHY 110 & PHY 110A, work experience in the physical therapy field or currently licensed Massage & Bodywork Therapist, bachelor's degree in an allied health related field, which includes exercise physiology, athletic training, orthotics, prosthetics, COTA, respiratory therapy, speech language pathology, etc. (Personal trainers, CNA, etc. are not included among "Allied Health"), associate degree in science, and the score on the ATI TEAS Test.
In the admissions process, the top applicants will be selected to continue to Phase II of the selection process for admission into the PTA program. Applicants will be interviewed and required to write an essay for Phase II of the application process. Applicants who successfully complete Phase II are ranked according to their performance in phases I and II.
Applicants with the highest rank on their grades earned in BIO 168, BIO 169, ENG 111, ENG 112 or ENG 114 and MAT 171, the ATI TEAS test, scores on letters of recommendation (one from and educator with experience teaching the prospective student within the past two years and one professional willing to recommend the student as a good candidate for the program), score on letter of intent from student, and score on interview with nursing faculty/administration committee will be admitted into the LPN-BSN program.
